These files are for you to download and Print out. The first three files are information that gets turned in by the coach at the time of registration, along with any payment. Parents may download these forms, print them out, and fill them out before giving them to the coach in order to save time.
Coaches:
1. Every child on your roster must have a completed and signed registration form.
2. Every child that has been awarded a scholarship by the manager must have a completed Scholarship Request Form in the registration folder when you turn it in.
3. Every coach associated with your team must have a completed application in your registration folder.
