Only players that have paid in full are to be listed on the roster and will be allowed to participate.
The head coach is responsible to make sure all players listed on the roster have paid. Players that have paid online should have a receipt to show the coach for verification.
The PAL staff will check all names on the roster prior to registration day. Unpaid players will be removed from the roster.
The Full Team Name, the Sport, the Year, and the Age Division are required fields. The Division is optional.

The Head Coach's Email Address must be listed in order to submit the form. Please use a valid email address - otherwise you may not receive the roster.
At least one player must be listed in order to submit the form. First Name, Last Name, and Date of Birth (DOB) are required fields per player. Address, City, and ZIP are optional.
